Appointment or change of a supervisor

Following § 12 (1) of the AGH Doctoral School Study Regulations, a doctoral student shall, within 30 days of undertaking the course of education at the School, apply for the appointment of a supervisor, supervisors or an assistant supervisor to the Chairman of the competent Scientific Discipline Board.

The application addressed to the Chairman of the Discipline Board by the Director of the Doctoral School should include:

  • student’s personal data, including name, PESEL (Personal ID number) and, in the absence of PESEL, full name and number of a document confirming the person’s identity,
  • a proposal of person to act as a supervisor(s) or supervisor and assistant supervisor, specyfing the disciplines and field in which the doctoral disssertation is to be conducted,
  • consent of the proposed person to take up as a supervisor(s) or an assistant supervisor,
  • consent of the head of the scientific unit - the Dean of the faculty.

Following § 12 (4) of the AGH Doctoral School Study Regulations, a doctoral student may apply for a change of the supervisor(s) or the assistant supervisor.

The application addressed to the Chairman of the Discipline Board by the Director of the Doctoral School should include:

  • student’s personal data, including name, PESEL (Personal ID number) and, in the absence of PESEL, full name and number of a document confirming the person’s identity,
  • name of the current supervisor (or assistant supervisor) and their consent to resign from the position of supervisor (applies to changes and cancellation),
  • name of the proposed new supervisor and their consent to take over the function,
  • justification of the application,
  • consent of the head of the scientific unit - the Dean of the faculty
